Drop-in centre opening times for 2008

Drop-in centres will be open for receipt of completed SPS application forms and supporting documents from 3 March to 9 June 2008, opening times:
Monday to Friday 8.30am – 5.00pm, closed Bank Holidays and weekends, except
Saturday and Sunday, 10 and 11 May, and 7 and 8 June, from 9.00am – 3.00pm.
A drop-in centre receipt will be issued.

Post boxes are available at drop-in centres for application forms to be deposited between 5.00pm and midnight, on 9 June. Receipts for forms deposited in the post boxes will be sent by post within ten working days.

Application forms deposited after midnight on 15 May will be subject to a late claim penalty. Application forms deposited after midnight Monday, 9 June will be rejected.

Please note that drop-in centre staff are able to take receipt of the application and supporting documents – they are not able to provide advice on form completion.

• Agent/Partnership authorisation form (SP9)
• Order of entitlements activation (SP11) – supplementary form for SP5
• Bank details registration form (SP12)
• Payment assignee details form (SP13)
