If an agent will be completing your SPS Application on your behalf you will need to complete the following Agent Authorisation form (SP9) and enclose it with your application form.

It is recommended that you check your printer settings before you print the blank form, in case the form is not printed correctly, i.e. not all character boxes and fields may be printed because they are outside normal printer boundaries.

To print out a copy:

1. Hold your mouse over the icon.

2. Using the 'right' mouse button click once.

3. Select the option 'Open Link in New Window'.

4. Print as normal.
